You can use this public lookup tool to find individual providers, specific agencies/facilities or to view all agencies/facilities for certain types.

Health facility licensing is a mandatory process that providers must go through in order to operate in Colorado. Certification through Medicare and Medicaid is an optional process that allows providers to bill for reimbursement. We do the entire process for licensing and the survey portion for certifications.

This site is used to register, notify, and inform individuals who are interested in volunteering in the event of an accidental or intentional emergency, or other public or health care disaster. This site can also be used to register and notify other industries in the event of an emergency.
